After You Send the Request

Booking online sends us a request. It is not a confirmed appointment until you hear from us.

We review it. Usually within one business day.

EZ Derm sends you a portal link by email or text, normally one to two days after the request is approved.

You fill out your forms there. Doing this in advance is what keeps a first visit short. We cannot log in to the portal for you, but the front desk can resend the link.

Before Your Visit

The answers people ask us for most. Anything else, call and we will talk it through.

Which insurance plans do you take?
We accept most major commercial insurers, local hospital system plans (OhioHealth, Ohio State, Mount Carmel), Medicare and Tricare. We do not accept Medicaid. We check your coverage against the insurers we work with before you come in, but insurers run a great many different plans and we cannot see the detail of yours. A quick call to the number on your card, confirming that we are in network and that the visit is covered, is the surest way to avoid a bill you were not expecting. This matters most on marketplace and narrow-network plans, which cover far fewer providers: a plan sold under a familiar insurer’s name may still not include us. HMO plans need a referral. The full list is on our insurance page.
What should I do before my first visit?
Complete your medical history, insurance details and intake forms in the portal before you come in, and arrive ten to fifteen minutes early. Bring your insurance card, a photo ID and a list of the medicines you take.
What if I need to cancel, or I am running late?
Please give at least 24 hours’ notice. A $25 fee may apply for a missed medical appointment and $75 for a missed surgical or cosmetic appointment. If you arrive more than 15 minutes late we may not be able to see you that day, and the visit may be recorded as a no-show. Exceptions are made for genuine emergencies, so call and tell us.
Why do you ask for a card on file?
It is the simplest way to settle a small balance left after insurance without an account drifting toward collections. We send at least three statements, by mail and electronically, before that point. If a balance is still unpaid after those, we may charge the card on file rather than send the account to an agency. You can pay a bill online at any time.
Can I get an estimate of the cost?
Yes. If you do not have insurance, or you are not using it for your visit, you can ask for a Good Faith Estimate before you come in. Call our office at 614-887-7723. Estimates come from us, not from the billing company.
How do I get a prescription refill?
Call 614-887-7723 and follow the prompts for non-urgent medical concerns. We are not able to refill a prescription if you have not been seen in the past year, or if the medication was not intended to be refilled. If either applies, we will help you book a visit.
I never got my portal link, or I cannot log in.
EZ Derm sends the link by email or text once a request is approved, so check your junk folder and your text messages first. We cannot log in to the portal on your behalf, but the front desk can resend the link. Call 614-887-7723.
Do I need a referral?
Most plans do not require one, but HMO plans do. Narrow-network and marketplace plans vary, so check with your insurer. If you are a referring provider, you can send us a patient through our referrals page.
